Step 5: Entering General and Discontinuation Information

 

1.      Left single-click on the ‘General’ tab.

2.      Enter the progress note date in the ‘Note Date’ field.

3.      Enter the Speech Therapy - SLP discontinuation date in the ‘ST DC Date’ field.

4.      Enter the reason for therapy discontinuation in the ‘ST DC Reason’ field.
